3. 3.All  survey responses are reported in-aggregate, e.g. “X% of respondents said ‘Yes’ to Question #1”
   

4. 4.For those research projects involving a focus group, telephone call interviews or other ways you may be personally identified: these are opt-in only meaning these conditions will be clearly stated up front and you will have the choice to participate or not. Only after you agree will you be asked to provide such information.
